If you are a patient, please note that this Policy is distinct from your Healthcare Provider’s HIPAA Notice of Privacy Practices, which describes how we or your Healthcare Provider use and disclose your protected health information, as well as any other privacy practices applied. Apex collects, uses, and discloses any Personal Information it receives from your Healthcare Provider in accordance with its HIPAA-required agreements with your Healthcare Provider.

This website and our services are intended for a United States audience. This Policy is incorporated into and made a part of the terms and conditions applicable to the use of this website (“Terms of Use”).   • SMS:  SMS opt-in and phone numbers for the purpose of SMS will not be shared with any third parties or affiliates for marketing purposes. If you wish to stop receiving text messages from us, reply STOP, Quit, Cancel, Opt-Out, or Unsubscribe to any text messages sent from us.
